Health Exchange Awarded £492k South Bristol Pre-Diabetes Programme Contract

NHS Bristol, North Somerset and South Gloucestershire Integrated Care Board has awarded Health Exchange a contract to deliver a community-based pre-diabetes programme in South Bristol.

The service will provide preventative support for adults aged 18 to 50 who have been diagnosed with pre-diabetes, with the aim of reducing progression to Type 2 diabetes, improving health outcomes and addressing health inequalities.

Health Exchange will provide a one-to-one assessment within six weeks of diagnosis and develop a personalised support plan. The programme will also provide education and self-management support to encourage sustainable lifestyle changes, followed by a review at six months to assess progress and outcomes.

The service will link individuals with local services across primary care, community provision and voluntary sector support. It will focus on engaging underserved populations and will operate across a range of community settings, including during evenings and weekends to maximise accessibility.

The contract is valued at £491,540.23 excluding VAT and £497,540.23 including VAT. It is expected to run from 18 January 2027 until 17 January 2030, with commissioners having the option to extend it for any period up to a further two years, potentially taking the contract through to 17 January 2032.

The award decision was made on 14 August 2026. The standstill period ends on 25 August 2026, with the earliest contract signing date stated as 1 September 2026.

Two tenders were received, with one assessed in the final stage. The final-stage tender was submitted by a voluntary, community and social enterprise. Health Exchange is identified as a VCSE and is not an SME.

The contract was awarded through an open procedure and is classified under Community health services. The procurement was carried out by NHS South, Central and West Commissioning Support Unit on behalf of the commissioners.
