The Council is exploring opportunities to develop an integrated preventative support service that combines hospital discharge support with specialist de-hoarding and home environment interventions.

The proposed service is intended to support residents whose health, wellbeing, independence or safety may be affected by their living environment, as well as individuals requiring practical support to return home safely following a hospital stay.

The Council has invited interested organisations to complete a market engagement questionnaire to help inform the future commissioning approach.
