NHS England to host free session on improving post-incident debriefs

NHS England is hosting a free virtual event this month exploring how co-production can be used to improve the way patients, carers and healthcare professionals are supported following incidents.

The Shine a Light on Co-production session will take place on 29 September 2026 from 10.30am to 12pm and will be held virtually through Microsoft Teams.

The event, titled “HEAR US: Enabling a purposeful post incident debrief,” will examine the development and implementation of the HEAR US and I am HEARD tools.

Both were created in partnership with people with lived experience, patients, carers and healthcare professionals, with the aim of improving post-incident debriefs and reducing restrictive practice.

Attendees will hear how the HEAR US framework is being used to support more meaningful and person-centred debriefing, allowing organisations to learn from incidents while improving the experiences of those involved.

The session will also introduce I am HEARD, a co-produced checklist designed to help patients, carers and loved ones understand what should happen following an incident and enable them to take an active role in the subsequent debrief.

NHS England says the tools are intended to strengthen partnership working, improve learning following incidents and support greater transparency and person-centred care.

The event is free to attend, with registered participants receiving the Microsoft Teams link 24 hours before the session.
