Leadership Rating Downgraded as Inspectors Raise Concerns Over Patient Safety, Governance and Complaints Handling
The Care Quality Commission (CQC) has issued a formal warning notice to County Durham and Darlington NHS Foundation Trust, requiring the organisation to make immediate improvements after inspectors identified concerns relating to patient safety, governance and leadership.
Following inspections carried out between February and April 2026, the regulator downgraded the trust’s overall leadership rating from Good to Inadequate, highlighting failures in governance, oversight and the management of patient complaints.
The warning notice requires the trust to address a number of areas where inspectors believed patients could be placed at unnecessary risk if improvements were not made promptly.
The inspection focused on leadership across the trust as well as surgical services provided at Darlington Memorial Hospital and University Hospital of North Durham.
While inspectors praised staff for their commitment to delivering compassionate care in often challenging circumstances, they concluded that weaknesses in leadership and governance were having an impact on the quality and safety of services.

County Durham and Darlington NHS Foundation Trust has acknowledged the CQC’s findings and says it has already begun implementing a comprehensive programme of improvements.
The trust highlighted that significant changes have taken place within its executive leadership team since the inspection and said a number of actions were already underway before the report was published.
These include strengthening governance arrangements, improving quality assurance processes, reducing complaint backlogs, enhancing oversight of patient safety and investing in leadership development across the organisation.
Trust leaders have said they remain fully committed to delivering safe, high-quality care and will continue working closely with the CQC to demonstrate sustained improvement.
