Cheshire West and Chester Council has awarded a contract to Cheshire Community Council to deliver its Community Home First service, supporting the timely discharge of patients from hospital and helping them return safely to their usual place of residence.
The service is designed to support Pathway 0 and Pathway 1 patients by embedding Community Sector Co-ordinators, known as Pathfinders, within hospital discharge teams. The Pathfinders will provide triage and coordination between hospital teams, community and voluntary sector organisations, and volunteers, helping to remove barriers to discharge and ensuring people receive appropriate support during the first few weeks after returning home.
The contract is valued at £736,800 excluding VAT (£921,000 including VAT) and is expected to run from 1 October 2026 until 31 March 2029.
The procurement was conducted using an open procedure under the light touch regime. A total of 22 tenders were received, with five progressing to the final assessment stage. All five final tenders were submitted by small and medium-sized enterprises (SMEs).
Through the Community Home First service, Cheshire Community Council will work alongside hospital discharge teams to coordinate voluntary and community sector support, helping patients recover safely at home following discharge.
