Bedfordshire Care Home Placed in Special Measures After Serious Safety Failings

The Care Quality Commission (CQC) has downgraded Woodland House care home near Toddington, Bedfordshire, from outstanding to inadequate overall and placed it in special measures following an inspection completed in June 2026.

Run by Really Flexible Care Ltd, Woodland House provides residential accommodation and personal care for autistic people and people with a learning disability. Three people were living at the service when inspectors visited. The inspection followed concerns about the safety and quality of care.

CQC found the service was breaching five regulations covering person-centred care, dignity, safety, safeguarding and management.

The home’s ratings for responsive and well-led care fell from outstanding to inadequate. Safe and caring were downgraded from good to inadequate, while effective fell from good to requires improvement.

Inspectors identified several safety concerns, including exposed wires, damaged walls, infection risks and restricted areas without evidence of appropriate review or authorisation. They also found that safeguarding concerns were not always identified or escalated consistently, and medical assistance had not been sought following repeated head injuries associated with self-injurious behaviour.

Incident records relating to bruising and unexplained injuries lacked sufficient detail. Inspectors also found shortcomings in the recording and monitoring of restrictive practices, alongside concerns about staff language, respect for people’s dignity and the support provided for meaningful activities and independence.

Further concerns included inadequate monitoring by leaders, weaknesses in coordination with healthcare partners, failures to consistently assess people’s decision-making capacity and insufficient protection of confidential information. Although care plans were detailed, they were not always personalised, accurate or reflected in the care delivered.

CQC deputy director of adult social care for the East of England, Hazel Roberts, said the inspection revealed complacency among staff and leaders, with missed opportunities to address risks to people’s safety and wellbeing. CQC also raised safeguarding concerns with the local authority for investigation.

The regulator said leaders had begun addressing the issues identified. Special measures will involve closer monitoring and a structured timeframe for improvements, with further action possible if the required changes are not made.

CQC has also started the process of taking regulatory action against the provider, which has the right to appeal. Inspectors will return to assess progress.
