Stoke on Trent City Council is seeking providers to deliver two specialist short-term home-based support services for adults aged 18 and over.
The services will support people during periods of increased need, crisis, recovery, assessment or transition, helping individuals remain safely at home while reducing unnecessary hospital admissions and long-term residential care placements.
A short-term overnight service operating between 11pm and 6am. Support will be delivered in individuals’ homes and may include supervision, observation and reassurance during periods such as hospital discharge, deterioration in health or carer breakdown.
The service will also provide professional observations to help inform future care planning.
Flexible, intensive short-term home-based support for people requiring regular monitoring and intervention throughout the day.
The service will help bridge gaps in provision, promote recovery and independence, and provide evidence to support longer-term care planning.
Providers may bid for either or both lots, but no provider can be awarded more than one lot.

Each lot will be awarded to a single provider through the Council’s light-touch open procurement procedure.
