Great Organisations Don’t Just Solve Problems – They Learn From Them

Every health and social care organisation experiences challenges.

  • An incident.
  • A complaint.
  • An audit finding.
  • An inspection recommendation.

Sometimes things don’t go exactly as planned.

What separates great organisations isn’t that problems never happen. It’s what they do next. The strongest services take time to understand what happened, identify the lessons and make meaningful improvements that reduce the chance of the same issue happening again.

That’s the purpose of a Lessons Learned Register.

Learning should never stop

Every experience provides an opportunity to improve. Sometimes the learning comes from an incident. Sometimes it comes from a near miss. Sometimes it comes from positive feedback that highlights something your team is doing exceptionally well.

The important thing is making sure those lessons aren’t forgotten. Recording them creates knowledge that benefits the whole organization, not just the people involved at the time.

Don’t just ask “What happened?”

After any significant event, many organisations naturally ask:

“What happened?”

That’s an important starting point. But it’s only the beginning. The more valuable questions are:

  • Why did it happen?
  • What contributed to it?
  • Could it happen again?
  • What have we learned?
  • What will we do differently?
  • How will we know the improvement has worked?

These questions move the conversation away from blame and towards continuous improvement.

Learning from inspections

Inspections are valuable opportunities to receive independent feedback. Whether the outcome is positive or identifies areas for improvement, every inspection offers learning. Recommendations shouldn’t simply be completed and filed away.

Instead, consider:

  • What worked particularly well?
  • Which improvements had the greatest impact?
  • Are there lessons that could benefit other teams or services?
  • How can we embed these improvements into everyday practice?

Capturing this information helps ensure valuable learning isn’t lost once the inspection has finished.

Good ideas deserve to be shared

Learning shouldn’t stay with one manager or one team. When positive changes are made, share them.

Discuss lessons during:

  • Team meetings.
  • Supervision sessions.
  • Quality and governance meetings.
  • Induction programmes.
  • Training sessions.
  • Staff newsletters.

Sharing knowledge creates stronger teams and encourages a culture where everyone contributes to continuous improvement.

Focus on action, not just reflection

Recording lessons is important. Acting on them is even more important.

Every lesson identified should lead to a practical question:

“What are we going to do differently?”

That might involve:

  • Updating a policy.
  • Improving a process.
  • Delivering additional training.
  • Introducing a new checklist.
  • Strengthening communication.
  • Reviewing responsibilities.

Small changes made consistently often have the greatest long-term impact.

To help organisations capture learning consistently, we’ve created a free Daily Round Lessons Learned Register.

The template allows you to record:

  • The source of the learning.
  • A summary of what happened.
  • Root causes and contributing factors.
  • Lessons identified.
  • Improvement actions.
  • Named owners.
  • Target completion dates.
  • Outcomes and impact.

Whether the learning comes from an incident, inspection, complaint, compliment, audit or quality review, the register helps ensure valuable knowledge is captured and acted upon.

Five questions every leadership team should ask

  1. What have we learned this month?

Building regular reflection into governance meetings keeps improvement at the heart of everyday practice.

  1. Have we shared the learning?

If only one person knows the lesson, the organisation hasn’t truly learned from it.

  1. Have we completed the agreed actions?

Every improvement should have a named owner, a realistic timescale and regular progress reviews.

  1. Did the change make a difference?

Success isn’t measured by completing an action—it’s measured by the positive impact that action has on people, staff and the service.

  1. What can we do even better next time?

Continuous improvement doesn’t end when one action is complete. Every lesson provides the foundation for the next improvement.

The Daily Round takeaway

The best organisations don’t define success by avoiding every challenge. They define success by how quickly they learn, adapt and improve.

  • Every incident.
  • Every inspection.
  • Every complaint.
  • Every compliment.
  • Every audit.
  • Every experience has something to teach us, if we’re prepared to listen.

Because outstanding care isn’t built on never making mistakes. It’s built on recognising opportunities to improve, sharing what we’ve learned and using that knowledge to provide safer, kinder and higher-quality care for everyone.

Every lesson captured today has the potential to improve tomorrow.
