Every health and social care service faces risk. Whether it’s staffing pressures, medication errors, falls, infection prevention, safeguarding concerns or business continuity, risk is an unavoidable part of providing care.

The goal isn’t to eliminate every risk. That’s simply not possible. The goal is to recognise potential risks early, understand their impact and put appropriate measures in place to reduce the likelihood of them affecting the people we support.

That’s exactly why every organisation should maintain an up-to-date Risk Register.

Risk management is everyone’s responsibility

When people hear the words Risk Register, they often think it’s something only the Registered Manager or senior leadership team needs to worry about.

In reality, good risk management belongs to everyone. Every member of staff plays a role in identifying concerns, reporting issues and helping to create a safer environment. From reporting faulty equipment to raising safeguarding concerns or highlighting staffing pressures, frontline teams are often the first to recognise emerging risks.

The sooner those risks are identified, the sooner action can be taken.

Don’t wait for something to go wrong

The most effective organisations don’t wait until an incident occurs before taking action. They ask questions such as:

  • What could go wrong?
  • How likely is it?
  • What would the impact be?
  • What controls are already in place?
  • Is there anything else we should be doing?

A good Risk Register tells a story

A Risk Register shouldn’t become a forgotten spreadsheet that’s only reviewed before an inspection. It should be a living document that evolves alongside your service. Each risk should clearly identify:

  • The risk itself.
  • The potential impact.
  • The likelihood of it occurring.
  • Existing controls.
  • Additional actions required.
  • Who is responsible.
  • When the risk will be reviewed.

Keeping this information current helps leaders make informed decisions and demonstrates strong governance.

Review risks regularly

  • Risks change.
  • Staffing levels change.
  • Services expand.
  • New technology is introduced.
  • Legislation evolves.

That’s why reviewing your Risk Register regularly is just as important as creating it.

Monthly reviews, quality meetings and governance discussions all provide opportunities to update risks, close completed actions and identify new ones. Risk management should become part of everyday leadership rather than an annual exercise.

Learn from incidents

Not every risk can be predicted. However, every incident provides an opportunity to strengthen your Risk Register. After an accident, complaint, safeguarding concern or near miss, ask yourself:

  • Was this risk already identified?
  • Were existing controls effective?
  • Do additional actions need to be introduced?
  • Are similar risks present elsewhere in the service?

  1. What have we learned recently?

Complaints, audits, incidents, inspections and feedback all provide valuable insight that can strengthen your approach to managing risk.
