Social Prescribing service – Conwy

Conwy County Borough Council, in partnership with Public Health Wales, is seeking to appoint an experienced provider to deliver a county-wide Social Prescribing Service for residents across Conwy.

The service will provide person-centred support that helps people improve their health and wellbeing by connecting them with community-based activities, groups, services and opportunities. It is intended to support prevention and early intervention, reduce social isolation and inequalities, strengthen community resilience, and help people address the wider social determinants of health.

The successful provider will deliver the service in line with the Welsh Government National Framework for Social Prescribing and the Core Standards and Values for Social Prescribing in Wales. The service will place a strong emphasis on addressing inequalities, Welsh language provision, trauma-informed practice, safeguarding, and achieving measurable wellbeing outcomes.

Priority support will be provided for:

• Older people.
• Individuals experiencing loneliness or social isolation.
• People living with long-term conditions.
• Unpaid carers.
• People experiencing mild to moderate mental health difficulties.
• Residents living in deprived and rural communities.

According to the contracting authorities, the procurement represents an opportunity to expand and further develop the existing social prescribing service in Conwy in line with the new Welsh Core Standards.

Contract Value – £500,256

Delivery Length – Initial three-year contract commencing in October 2026, with the option to extend for up to a further two years until 30 September 2031.

The procurement will be conducted as a below-threshold open competition under the Light Touch Regime.
