Short Breaks Services Framework (CSC25128)
Worcestershire County Council
Location: North East
Darlington Borough Council is seeking a provider to deliver Provision of Personal Care and Support to Tenants in Extra Care Housing, providing regulated personal care and domestic support for individuals living within Extra Care Housing schemes across the borough.
The service is intended to support people with assessed care and support needs to remain living independently within Extra Care Housing, reducing reliance on residential care while promoting independence, wellbeing and community participation. The Council’s vision is to ensure people receive the right care at the right time and are supported to contribute to and feel valued within their local communities.
The service will be available to a wide range of individuals, including older people aged 55 and over, as well as younger adults whose Care Act assessment identifies Extra Care Housing as the most appropriate setting. The service will also support people living with dementia, physical disabilities, sensory impairments, long-term conditions, learning disabilities, mental health conditions, bariatric needs, palliative care needs, behaviours that may challenge, and carers whose assessment identifies a need for additional support.
The Council confirmed there are currently four Extra Care Housing schemes operating across Darlington, providing a total of 154 accommodation units.
The schemes comprise a mixture of Council-owned and social rented accommodation. Darlington Borough Council Housing operates three schemes and works in partnership with Anchor Housing on an additional scheme.
