Short Breaks Services Framework (CSC25128)
Worcestershire County Council
Location: South Wales
Bridgend County Borough Council is seeking a provider to deliver an Older Person’s housing-related support service for vulnerable older people across the county borough.
The contract will bring together three elements of support within one seamless service: Older Person’s Floating Support in the community, Hospital Discharge Support and support within Extra Care schemes.
The service will primarily support people aged 55 and over, although the successful provider will be expected to remain flexible in accepting referrals for people aged 50 and over where demand arises and capacity allows.
Housing-related support will focus on helping people maintain their independence, sustain their tenancy or home and reduce the likelihood of requiring more intensive support at a later stage. Delivery will include support within the community, the county borough’s sheltered accommodation and its three Extra Care schemes.
Support will be solution-focused, helping individuals resolve housing-related issues, prevent crises and settle or remain within their communities.
The Council is also seeking innovative approaches to meeting increasing demand for older people’s support services.
The service will contribute to the Housing Support Grant vision of preventing homelessness and enabling people to have safe homes where they can live independently. It will also support the Council’s responsibilities under the Housing (Wales) Act 2014, Social Services and Well-being (Wales) Act 2014 and Well-being of Future Generations (Wales) Act 2015.
The contract encompasses three areas of service delivery:
The Council has set a financial envelope of £500,000 per annum for the contract.
The opportunity is identified as particularly suitable for small and medium-sized enterprises (SMEs) and voluntary, community and social enterprises (VCSEs).
