Home Not Hospital: VCS Support for Admission Avoidance, Discharge, and Recovery

North East Lincolnshire Borough Council is inviting bids from voluntary and community sector (VCS) organisations to deliver support services that help prevent hospital admissions, facilitate safe discharge, and support recovery across North East Lincolnshire.

The service is intended to reduce avoidable hospital admissions while promoting independence through two initiatives: Enablement Champions and Home from Hospital/Reablement. The procurement is being undertaken through a below-threshold open competition under the Light Touch Regime.

Participation is restricted to voluntary, community and social enterprises (VCSEs) with substantive business operations based within the administrative area of North East Lincolnshire Council.
