The London Borough of Hammersmith & Fulham (LBHF) has launched a procurement for the delivery of Community Reablement & Home Care Services for Adults in Hammersmith & Fulham. The new framework is designed to support adults to regain or maintain their independence through a redesigned service model that places greater emphasis on reablement, strengths-based practice and community connections.
The procurement reflects the Council’s strategic redesign of its previous service model, with services focused on helping residents regain skills, confidence and independence while reducing hospital admissions, readmissions and the need for long-term care.
The framework has been divided into two lots:
Lot 1 provides short-term, intensive reablement support for adults following illness, hospital discharge or other crises. The service focuses on enabling people to relearn daily living skills, maximise independence and reduce reliance on ongoing care.
Lot 2 covers longer-term homecare support for adults requiring ongoing assistance to remain safely at home. Services include personal care and daily living support delivered in line with assessed needs and individual care plans.
The framework will operate using a multi-lot locality model, with each locality appointing Primary, Secondary and Tertiary Providers to receive care packages through a call-off process.

The framework will appoint a maximum of 18 providers, with three providers allocated to each locality as Primary, Secondary and Tertiary Providers.
Potential providers may bid for one or both lots, although only one locality may be awarded under each lot.
The procurement is intended to strengthen community-based support by combining reablement and homecare services within an integrated model focused on promoting independence and preventing the need for long-term care wherever possible.
