Cambridgeshire County Council is seeking to commission a Warm Homes Single Point of Contact (SPoC) service to provide an integrated health and housing referral pathway for vulnerable people living in cold and fuel-poor households.

The service will act as a single access point for referrals from both individuals and professionals, providing integrated assessments and tailored onward interventions. The aim is to ensure vulnerable residents receive timely, effective and appropriate support that helps mitigate the risks associated with cold homes and winter weather on health and wellbeing.

Service Overview

The appointed provider will deliver a Warm Homes Single Point of Contact service designed to:

• Accept referrals from both self-referrals and professionals.
• Provide integrated health and housing assessments.
• Deliver tailored onward interventions.
• Support vulnerable people living in cold and fuel-poor households.
• Reduce the health and wellbeing risks associated with cold conditions and winter weather.
