A new NHS service has opened at Haslemere Hospital to identify deteriorating health earlier and help people with long-term conditions remain independent.
The Haslemere Proactive Care Hub brings several healthcare professionals together so patients can receive a comprehensive assessment during one visit. Local GPs will identify people who may benefit and refer them for an initial review and follow-up checks.
The service is initially focused on patients living with heart failure. During appointments lasting between two and three hours, patients can see a cardiologist, receive strength and falls-risk assessments from a therapist, and undergo medication reviews and general health checks with a GP-led team.
Each patient will receive a personalised care plan, while a social prescriber is expected to join the service as it develops. This will help connect people with community support addressing wider health, wellbeing and social needs.
Royal Surrey NHS Foundation Trust hopes earlier intervention will prevent manageable changes from becoming serious health problems requiring hospital care. The hub plans to expand into respiratory illness, diabetes and neurodegenerative conditions from early 2027, with the model potentially introduced elsewhere across West Surrey.
