Stoke-on-Trent City Council is seeking providers to deliver NHS Health Checks through a community outreach model, expanding access beyond general practice to improve uptake among communities with low coverage, high health needs, or historically low engagement.
The NHS Health Check programme is a statutory service for adults aged 40 to 74 who do not have a diagnosis of heart disease, kidney disease, stroke, or type 2 diabetes. The service provides assessments to identify an individual’s future risk of cardiovascular disease and related conditions through a range of health measures, including blood pressure, cholesterol testing, height, weight, smoking history, and physical activity.
The Council advises that the programme was suspended during the COVID-19 pandemic and continues to recover. The procurement aims to increase delivery through non-healthcare settings by working with local communities and adopting targeted approaches to improve participation.

The Council has confirmed that payment will be made through a hybrid model consisting of a fixed set-up payment for each event or venue alongside activity-based payments for completed health checks.
