Lambeth Council is seeking a provider to deliver a Community Reablement Support Service as part of its integrated reablement pathway, supporting residents to regain independence following illness, injury or a hospital admission.
The successful provider will supply reabling home care workers who will work alongside clinical staff from Guy’s and St Thomas’ NHS Foundation Trust and social workers and assessors from Lambeth Council. The service is designed to provide short-term support that helps individuals rebuild confidence, maximise independence and reduce ongoing care needs.
According to the Council, the provider will be required to maintain a presence at the Intermediate Care Lambeth base at Ackerman Health Centre and participate in regular multidisciplinary team meetings. The service will operate across the full geographical area of the London Borough of Lambeth.
The contract value is based on an hourly rate for support delivered, with an indicative annual volume of 28,500 hours.
The Community Reablement Support Service will provide short-term home-based support to individuals following periods of ill-health or hospital admission.
Reablement workers will form part of an integrated multidisciplinary service, working closely with health and social care professionals to support recovery, independence and positive outcomes for residents.
The Council has indicated an expected delivery volume of approximately 28,500 support hours per year.
