The Care Quality Commission (CQC) has downgraded Allot Healthcare Services York to Inadequate and placed the domiciliary care service in special measures following an inspection in August.
The service, operated by Allot Healthcare Services Ltd, provides personal care to people in their own homes. The CQC report records an overall rating of Inadequate dated 2 October 2026.
CQC downgraded the service’s safe rating from Good to Inadequate, while effective and well-led both fell from Requires Improvement to Inadequate. The caring and responsive ratings declined from Good to Requires Improvement.
Inspectors identified concerns about leadership, communication, staffing consistency and timekeeping. CQC reported that leaders had failed to address recurring issues highlighted in the service’s own surveys, complaints and audits. Relatives also raised concerns about difficulties contacting the office, changes to visits and access to familiar carers.
The regulator found that safeguarding incidents, including injuries, had not always been escalated to the relevant authorities. In one case, a person experienced a head wound and three other health episodes on the same day without the incidents being escalated.
CQC also identified gaps in staff training and knowledge, inaccurate or outdated care records, and failures to assess people’s needs and risks adequately when they joined the service. Inspectors found that risks relating to seizures, falls, skin deterioration, nutrition, bowel health and pressure care were not always recognised or reviewed effectively.
Further concerns included unsafe or delayed medication administration, inadequate information about allergies and adverse reactions to medicines, and failures to maintain buildings and equipment safely.
CQC said the service would remain under close monitoring while it assessed whether sufficient improvements had been made. The regulator has also begun the process of taking further regulatory action, which Allot Healthcare Services Ltd has the right to appeal.
