Wales to end automatic hospital follow-ups in £145m drive to cut waiting lists

Patients in Wales will no longer automatically receive routine follow-up appointments after hospital treatment under a £145 million plan designed to free specialists to see more new patients and carry out more procedures.

The Welsh Government has announced a major overhaul of outpatient care as part of efforts to reduce NHS waiting lists, with health boards expected to move away from routinely booking patients back into clinics when there is no clear clinical need. Instead, specialists will determine whether another appointment is necessary, while greater use will be made of patient-initiated follow-up arrangements. These allow people to contact the service if their symptoms return or their condition changes rather than attending appointments at fixed intervals regardless of whether they need further care.

Patients who require ongoing monitoring or whose clinicians believe a follow-up is necessary will continue to receive appointments. The Welsh Government believes reducing unnecessary follow-ups will release significant amounts of clinical time, allowing consultants and other specialists to concentrate on patients waiting for their first appointment, diagnostic procedures and treatment.

The changes form part of a £145 million investment intended to increase NHS activity and reduce some of the longest waits in Wales. Health boards will also be expected to increase productivity in operating theatres and outpatient departments, make greater use of evening and weekend capacity and improve the way patients move through planned care.

Waiting times have remained one of the biggest pressures facing NHS Wales, with thousands of patients experiencing lengthy delays for specialist treatment. The government says changing the way follow-up appointments are managed could help make better use of existing NHS capacity rather than simply adding more appointments to already stretched clinics.

The approach also reflects a wider shift towards giving patients greater control over when they return to hospital. Patient-initiated follow-up systems are already used for some conditions across the UK and are intended to prevent people attending appointments that provide little clinical benefit while allowing them to seek help when it is actually needed. However, the success of the policy will depend on patients understanding how and when they can return to specialist care and services being accessible when they do.

For NHS Wales, the aim is to create more capacity without compromising care for people who genuinely need continued monitoring. Ending the assumption that every hospital appointment must automatically lead to another is now being positioned as one way of doing that.
