Digital Diabetes Prevention and Primary Hypertension Programme

NHS Scotland intends to establish a national Framework Agreement for a Digital Diabetes Prevention and Primary Hypertension Programme, with an estimated total value of £4.8 million excluding VAT.

The programme will provide evidence-based, digitally enabled lifestyle and behaviour change interventions for people at increased risk of developing Type 2 Diabetes and individuals diagnosed with primary hypertension.

A single supplier will be appointed to the national framework, delivering services across Scotland through an integrated digital platform supported by appropriately trained personnel, health coaching and clinical governance arrangements.

Service Pathways

The requirement comprises one integrated service covering two pathways:

  • Digital Diabetes Prevention Programme (DDPP) – supporting individuals identified as being at increased risk of developing Type 2 Diabetes
  • Primary Hypertension Programme (PHP) – supporting people diagnosed with primary hypertension through lifestyle and behaviour change interventions designed to improve health outcomes and reduce cardiovascular risk

The service will be designed to help participants achieve sustainable improvements in health behaviours and clinical outcomes.

Key Service Requirements

The appointed provider will use a digital platform or application alongside trained personnel to support participant engagement, education, self-management, monitoring and behaviour change.

Provision is expected to include, where appropriate:

  • Digital onboarding
  • Risk assessment
  • Lifestyle interventions
  • Health coaching
  • Educational resources
  • Progress monitoring
  • Outcome reporting
  • Onward referral or escalation where clinically indicated

The solution must be accessible and inclusive for diverse populations across Scotland, including people who may face barriers associated with geography, deprivation, language, disability or digital exclusion.

The provider will also be expected to work collaboratively with NHS Scotland, NHS Boards, Primary Care and other relevant stakeholders across areas including programme promotion, participant recruitment and self-registration, mobilisation, delivery, performance monitoring and continuous improvement.

Expected Participant Volumes

Current funding has been identified to support approximately 15,000 participants through the Diabetes Prevention pathway during the initial three-year funded period.

The Primary Hypertension pathway is anticipated to support approximately 500 participants per year.

These figures are indicative and do not represent guaranteed volumes.

Subject to future funding and service requirements, NHS Scotland may increase participant volumes or expand the framework to include additional prevention cohorts, pathways, clinical indicators or service requirements closely aligned with the framework’s existing scope.

The £4.8 million estimated value relates to the full four-year framework period. NHS Scotland has confirmed that only three years of funding are currently secured and that additional purchases and participant volumes may be commissioned if further funding becomes available.

Evaluation Criteria

Tenders will be evaluated using the following weighting:

  • Quality – 60%
  • Commercial – 40%

Community benefits are also included within the requirement. The successful supplier will be required to outline its existing community benefits and proposals and support the associated contract and supplier management process following award.

Provider Requirements

Suppliers will initially be assessed through the Single Procurement Document (SPD) and Pre-Qualification Questionnaire (PQQ).

The selection stage will assess areas including:

  • Supplier suitability
  • Technical and professional capability
  • Capacity
  • Relevant experience

A maximum of five suppliers will be shortlisted and invited to the Invitation to Tender stage.

Specific economic and financial standing criteria and detailed technical requirements are contained within the procurement documents.

Procurement Procedure

The procurement is being conducted through a restricted procedure and will establish a framework agreement with a single supplier.

Following the initial selection stage, a maximum of five shortlisted suppliers will progress to the Invitation to Tender stage.

The estimated date for issuing invitations to tender is 8 October 2026.

The opportunity represents a significant national digital health commissioning programme, combining diabetes prevention and primary hypertension support within a single service model.
