Tower Hamlets is seeking a provider to deliver its Integrated Information, Advice and Advocacy Service, which acts as the front door for adult social care.
The service provides information, advice and general advocacy relating to social care, health, wellbeing and social welfare, offering early help to residents who often have multiple needs, including financial difficulties, health needs and social care support.
Acting as the initial point of contact for adult social care, the service provides an effective screening and support function to help manage demand. By delivering timely information and advice, directing residents to appropriate community services and supporting people to maximise their independence, the service aims to improve outcomes while reducing the need for more intensive social care interventions and helping to reduce waiting times for social care support.
The contract will be awarded through a competitive tender.
