Fife Council’s Health & Social Care Partnership is seeking a provider to deliver care and support services for four individuals aged 16 and over living in two neighbouring properties in Oakley.

The service will support individuals with learning disabilities, severe and enduring mental illness or disorders, and/or physical disabilities who require specialised support to remain in their current accommodation.

Service Overview

The successful provider will deliver care and support services for a group of four individuals.

The service will support:

• Four individuals aged 16 and over.
• Individuals with learning disabilities.
• Individuals with severe and enduring mental illness or disorders.
• Individuals with physical disabilities.

The individuals require specialised support due to needs which disrupt significant areas of their lives, including:

• Relationships.
• Employment.
• Housing.
• Self-care.

The accommodation comprises two properties located on the same street in Oakley, with two individuals living in each property.
