NHS Arden and Greater East Midlands Commissioning Support Unit (AGCSU) is seeking to appoint a provider to deliver the lifestyle and clinical elements of the Community Health Improvement and Lifestyle Service across Coventry and Warwickshire.
The Commissioner is looking to appoint a single accountable provider responsible for delivering an integrated service that supports people to achieve and maintain a healthier weight. The successful provider may deliver services directly or work in partnership with Voluntary, Community, Faith and Social Enterprise (VCFSE) organisations, primary care providers and community pharmacies, while retaining overall responsibility for service delivery and outcomes.

The successful provider will deliver a comprehensive, integrated and evidence-based Community Health Improvement and Lifestyle Service that forms part of a wider integrated care pathway.
The service must provide:
Lifestyle and behavioural support must form the foundation of the service, including support for people who are not eligible for, or choose not to receive, pharmacotherapy. All aspects of the service must operate as a single coordinated pathway to deliver safe, effective and equitable care.

Providers will be expected to maintain appropriate insurance arrangements, including employer’s liability, public liability and professional indemnity insurance.

The service must be delivered across a range of community settings, with face-to-face provision prioritised in neighbourhoods with the greatest identified need while also offering digital and remote delivery options to improve accessibility.
Providers will be expected to deliver inclusive services that respond to differing levels of clinical need and personal circumstances, ensuring reasonable adjustments and accessible communication are available to reduce health inequalities.
The successful provider will also be required to utilise the Integrated Care Board’s commissioned digital platform and population health data to support patient identification, care coordination, monitoring and service reporting.
This opportunity provides experienced healthcare providers with the chance to deliver an integrated lifestyle improvement service that supports healthier weight management and wider prevention objectives across Coventry and Warwickshire, working collaboratively with health, primary care and community partners.
