All Age Carers Support Service CRN/2026/57

Stoke-on-Trent City Council, working with Health, is seeking a provider to deliver an All Age Carers Support Service that will provide early identification, information, advice and tailored support to unpaid carers across the city.

The service is designed to improve the health, wellbeing and resilience of unpaid carers, helping them to sustain their caring role through accessible and inclusive support. The successful provider will work collaboratively with health, social care, and voluntary and community sector organisations to promote prevention, early intervention and improved outcomes for carers of all ages.

The commissioned service will include:

  • Early identification of unpaid carers.
  • Information, advice and tailored support.
  • Carers’ assessments.
  • Wellbeing support.
  • Peer support networks.
  • Effective signposting to appropriate services.
  • Partnership working with health, social care, and voluntary and community sector organisations.
  • A coordinated approach to improving outcomes for carers of all ages.
  • Support focused on prevention and early intervention.

The contract is expected to commence on 1 April 2027 and run until 31 March 2028, with the option to extend annually for up to a further four years at the Council’s discretion.
